Why Parent ER Ready exists

Parent ER Ready helps adult children organize the information they may need to communicate when an aging parent reaches the Emergency Room. It was created around a simple problem: vital answers are often scattered across medication bottles, phones, folders and family members when time and attention are limited.
What we make
The Self-Guided Bundle is a fillable and printable family organization system. It brings medications, allergies, conditions, emergency contacts, usual cognition and mobility, document locations and current-event notes into a focused handoff. Guided Setup is a separate service being developed to help families organize their own confirmed answers one step at a time.
Who it is for
The tools are designed for family caregivers and adult children preparing information for an older parent. They can also help families agree on one current master copy and make the most important pages easier to find.
Our scope
Parent ER Ready is an information-organization product, not a medical provider. It does not diagnose, recommend treatment, verify medical accuracy, create an official medical record or replace emergency assessment. Families should verify uncertain information with the appropriate clinician or pharmacist and should never delay emergency care to complete a form.
How our guides are prepared
Our free family guides use public material from government health agencies and professional organizations, then translate it into practical organization steps. Each guide names its sources and explains its limits. Read our editorial policy for the complete process.
